Patient Navigation

Patient navigation is a culturally tailored intervention to help individuals navigate through the complex cancer healthcare labyrinth from outreach to survivorship.

We have trained patient navigators who are:

• Aware of social, financial and cultural barriers to care
• Ready to work with people of all socioeconomic levels
• Organized and can solve problems effectively
• Able to locate resources for patients
• Understanding of the culture and religion of the community being served
